Iran?s Supreme Leader Tightens Grip After Disqualifying Two Top Presidential Candidates

For the cleric who runs Iran, there?s no such thing as a pleasant surprise, especially on election day. Ayatollah Ali Khamenei was not pleased when a librarian named Mohammad Khatami was swept into the president?s office in 1997, leading a wave of reformists who challenged the status quo in which Khamenei, as the unelected Supreme Leader of the Revolution, was most heavily invested. In every election cycle since, the self-appointed portion of Iran?s government has done all it can to winnow the choices placed before Iranian voters. On Tuesday, that system tightened the screen once more,?disqualifying the only two prominent candidates who dared to differ with the Supreme Leader. When Iranians go to the polls on June 14 to choose a successor to Mahmoud Ahmedinejad, the ballot will run from Khamenei?s former policy director to the man who married his daughter.

?We see in retrospect, the system was absolutely terrified at the possibility of a Gorbachev phenomenon,? says Ray Takeyh, who follows Iran at the Council on Foreign Affairs in Washington. He refers to Mikhail Gorbachev,? who engineered the demise of the Soviet empire its politburo selected him to protect. ?Because of Khatami. They don?t want a repeat of that performance.?

Consider the two most prominent candidates barred by the unelected Guardian Council, ?the last two guys who weren?t going to completely tow the line,? says Takeyh. Akbar Hashemi Rafsanjani, 78, has served twice as Iran?s president already, in 1989 and 1993. He was an architect of the Islamic Revolution that brought Grand Ayatollah Ruhollah Khomeini to power 34 years ago. At one point, when Khomeini thought he was about to be killed, he wrote a will ordering payment of his debts ? Rafsananji was owed the most.

Khamenei owes him a good deal more. After Khomeini?s 1989 death, the assembly of clerics charged with naming a successor was deadlocked until Rafsanjani announced he?d heard Khomeini say Khamenei would be good in the job. But not even that bit of history was enough to undo Rafsanjani?s association with the reform movement that later rose in reaction to the harshness of the mullah?s rule. ?It?s a little bit like Kramer versus Kramer now in the Iranian regime,? says Meir Javedanfar, an Iranian-born analyst in Tel Aviv. ?The regime is turning against itself.?

Less surprising was the disqualification of Esfandiar Rahim Mashaei, the man Ahmedinejad named as his vice president until Khamenei ordered him out of the job in 2009. Mashaei was dubbed part of a ?deviant current? after suggesting that perhaps clerics are not necessary to mediate between ordinary people and God. He?s also spoken encouragingly of Iranian nationalism, a potent residual force in a nation that predates the arrival of Islam by more than a millennium. ?And everybody knows the Leader can?t stand him,? says Takeyh.

His association with Ahmedinejad underscores the perils of elections. Loathed by the West for denying the Holocaust, Ahmedinejad looked like the savior of the Iranian establishment when he burst onto the national stage in the 2005 presidential election. In Iran?s elitist politics, his populist appeal was real and profound and Khamenei embraced him ?almost as the son he never had,? says Javedanfar. The Leader mobilized the establishment to assure the young ideologue was re-elected in 2009, allowing the appearance of vote fraud so massive it brought hundreds of thousands of Iranians into the streets, and left scores dead. But in the second term Ahmedinejad challenged Khamenei?s authority repeatedly ? and more directly than Khatami ever dared. The mild-mannered reformist was so averse to confrontation that in the moment of one election showdown he was admitted to the hospital, complaining of back spasms.

?They?re trying to do the election this time completely different from 2009,? says Takeyh. Security has been stepped up, and the internet slowed down. ?What they want to do is get through this electoral cycle with maximum degree of public apathy.? The field of candidates should help there. The Leader?s reported favorite is Saeed Jalili, who before heading the negotiating team on Iran?s nuclear program ran Khamenei?s office. Also approved to run was former foreign minister Ali Akbar Velayati, Tehran mayor Mohammad-Bagher Ghalibaf, another former nuclear negotiator, Hassan Rowhani, and former parliament speaker and Khamenei son-in-law Gholam Ali Haddad-Adel.

It is a not a diverse field. As the screening of candidates has grown more exacting, the base of Iranian politics has narrowed to a thinness that analysts describe as perilous. The press, which a decade ago was still lively, now reflects a political spectrum running from A to B: ?Principlists? square off against ?traditional conservatives.? Everyone else is on the outside peering in. ?The smaller the circle becomes, the worse the legitimacy,? says Javedanfar, who lectures on Iran at the Inter-Disciplinary Center north of Tel Aviv. ?There?s also the fact that those who are in are not the sharpest tools in the box.?

On the other hand, it may not be a great time for the mullahs to court unnecessary risks. ?Iran?s economy is crippled by international sanctions aimed at diminishing its nuclear program, and Tehran?s only military ally ? Syrian strongman Bashar Assad ? is fighting for his life, along with that of the Hizballah militia that Iran sponsors in adjacent Lebanon.

?They may actually succeed in stage-managing this thing. They threw out the two candidates who potentially could have caused trouble,? says Takeyh. ?But you never know. Who?d have predicted in 2009 that a has-ben like Mir-Hossein Mousavi would have inspired massive street protests? In 2009 they invited 400 foreign journalists into the country to watch. Did they expect things to go wrong??

Calcium-rich diet and supplements provide similar benefits

Taking a calcium supplement of up to 1,000 mg per day can help women live longer, according to a study whose lead author was Lisa Langsetmo, a Ph.D. Research Associate at McGill University, and whose senior author was Prof. David Goltzman, Division of Endocrinology and Metabolism in the Department of Medicine of the Faculty of Medicine and researcher in the Musculoskeletal Disorders axis at the Research Institute of the McGill University Health Centre (RI-MUHC).Their findings are published in the Journal of Clinical Endocrinology & Metabolism (JCEM).

Calcium, an essential nutrient for bone health, is commonly found in dairy products as well as vitamins. Although calcium is an essential nutrient for bone health, past studies have linked calcium supplements to heart disease risk. The researchers, located at universities across the country, analyzed data from the large-scale Canadian Multicentre Osteoporosis Study (CaMos) seeking to determine whether calcium and vitamin D intake were associated with overall increased risk of death.

"We found that daily use of calcium supplements in women was associated with a lower risk of death, irrespective of cause," said the study's lead author, Prof. Goltzman, Director, Calcium Research Laboratory at McGill. "The benefit was seen for women who took doses of up to 1,000 mg per day, regardless of whether the supplement contained vitamin D."

The longitudinal study of participants living in or near 9 cities across Canada monitored the health of 9,033 Canadians between 1995 and 2007. During that period, 1,160 participants died. Although the data showed women who took calcium supplements had a lower mortality risk, there was no statistical benefit for men. The study found no conclusive evidence that vitamin D had an impact on mortality.

"Higher amounts of calcium were potentially linked to longer lifespans in women, regardless of the source of the calcium," says Goltzman. "In other words, the same benefits were seen when the calcium came from dairy foods, non-dairy foods or supplements."

Regeneron, Sanofi asthma drug seen as potential game changer

(Reuters) - A new type of asthma drug meant to attack the underlying causes of the respiratory disease slashed episodes by 87 percent in a mid-stage trial, making it a potential game changer for patients with moderate to severe disease, researchers said on Tuesday.

"Overall, these are the most exciting data we've seen in asthma in 20 years," said Dr. Sally Wenzel, lead investigator for the 104-patient study of dupilumab, an injectable treatment being developed by Regeneron Pharmaceuticals Inc and French drugmaker Sanofi.

The drug also met all its secondary goals, such as improving symptoms and lung function and reducing the need for standard drugs called beta agonists.

Although far larger trials will be needed to confirm findings from the "proof of concept" study, researchers expressed optimism. They noted that dupilumab has also shown the ability to tame atopic dermatitis, or severe eczema, an allergic condition that is not well controlled by current treatments.

Results of the 12-week asthma study are being presented on Tuesday at the annual scientific meeting of the American Thoracic Society in Philadelphia.

The medicine, if approved, could hold promise for patients with moderate to severe persistent asthma that is not well controlled by standard drugs.

"We have been treating asthma with sort of Band-Aid therapies that didn't get at the underlying causes," Wenzel said in an interview, adding that dupilumab could be an important step in going to the root of the problem.

The drug works by simultaneously blocking proteins that have been linked to inflammation, interleukin-4 (IL-4) and interleukin-13 (IL-13).

HITS ELUSIVE TARGETS

Wenzel, director of the Asthma Institute at the University of Pittsburgh, said other drugmakers have tested medicines that block one or both of the proteins, but without success.

The trial recruited patients with high levels of eosinophils, a biomarker that shows immune system cells called type 2 helper T cells (Th2 cells) associated with allergy and asthma have been activated.

Such patients were deemed likely to benefit from treatment.

All patients initially stayed on their standard asthma treatments, meaning medium-to-high doses of inhaled glucocorticoids, as well as long-acting beta agonists. But patients gradually tapered off on those drugs and were no longer taking either of them after 9 weeks.

Throughout the Phase IIa trial, half the patients also received weekly injections of dupilumab, while half received placebo injections.

After the ninth week, 25 percent of those on placebos had experienced exacerbations, a catch-all term that included the need to take a beta agonist, a decrease in lung function, the need for an oral or inhaled corticosteroid, or if the patient went to the hospital or emergency room for worsening asthma.

"By end of the trial, after 12 weeks, 44 percent of those in the placebo group had exacerbations, compared with 5 percent of those on dupilumab," Wenzel said. That represented an 87 percent reduction in exacerbations, which was highly statistically significant.

Wenzel said dupilumab was well tolerated, with side effects similar to placebo. But she cautioned that longer trials are needed to fully assess the drug.

Regeneron and Sanofi said standard drugs are unable to control asthma in 10 to 20 percent of patients. They estimate that inflammation caused by Th2 cells - the type of inflammation among patients they tested - affects up to 2.5 million people in the United States and up to 30 million worldwide.

Dupilumab has also shown strong hints of safety and effectiveness in two early-stage trials that involved 67 patients with atopic dermatitis. Larger studies are slated to begin later this year.

Atopic dermatitis is inherited and involves patches of highly itchy skin on any part of the body. Patients, many of whom also have asthma and hay fever, have compared the sensation to having unending poison ivy.

"This asthma data and the data we already have in atopic dermatitis really raises the possibility the scientific community has finally hit upon the key pathway across all these allergic diseases," George Yancopoulos, Regeneron's research chief, said in an interview.

Trial results were also reported on Tuesday in the New England Journal of Medicine, along with an editorial that said results of the study appeared "compelling" at first glance and superior to those seen in previous studies of other asthma drugs.

But the editorial said effectiveness of dupilumab has been established in just a "limited subpopulation of patients with asthma" because only 21 percent of those screened for enrollment in the study met its criteria.

"We do not know whether dupilumab will be effective in other patient populations," the editorial said, including patients that stay on their standard therapies and who do not have high levels of eosinophils.

The sci-fi sequel directed by J.J. Abrams earned $70.1 million in its opening weekend. The original "Trek" reboot starring fresh faces as the crew of the Enterprise opened with $75.2 million in 2009.
